To get Skull Shaker shotgun in Resident Evil 4, you have to purchase the Deluxe Edition of the game or upgrade to the Extra DLC Pack. Unlike LE 5 or Red9, you can't get the Skull Shaker shotgun in-game or purchase it from the Merchant. 

As of this writing, the only way to get your hands on this unique shotgun is via real-life money. However, buying the Extra DLC Pack isn't the only way to get this gun in the game. You can also purchase the Skull Shaker shotgun from the PSN/Xbox/Steam store individually for just $2.
