Spider-Man 2 features over a dozen villains, including the likes of Mr. Negative, Kraven, and Venom. While they have been part of the game's promos and trailers and feature heavily in the main story, others pop up during side quests and are easy to miss.
With Venom being one of the main villains and the Symbiote being a huge part of the story, it's not far-fetched to wonder whether Spider-Man's other Symbiotic nemesis, Carnage, makes an appearance or is referenced in this sequel.

Does Carnage Appear In Spider-Man 2? Answered
Yes, Carnage is in Spider-Man 2, though not in his Symbiotic form. Carnage appears as the leader of the Flame Cult, whom you encounter in the Flame side missions.
In the final mission of the Flame, their leader gets hold of a part of the Symbiote and talks about bringing Carnage to the city. While there's nothing more, it's likely a hint of what's to come in a possible sequel.
If the finale of the Flame story arc in Spider-Man 2 is anything to go by, Carnage could be a major villain in Spider-Man 3.
As of this writing, we don't know if there even will be a Spider-Man 3, as Insomniac's next game is Wolverine. However, if Spider-Man 3 eventually happens, you can probably add Carnage at the top of the long list of villains that are likely to appear.
