The Wheel of Pain is a mechanism primarily used to convert human NPCs into Thralls which then can be assigned either for combat, companion, or crafting purposes. Every Thrall has four tiers needed to unlock by placing them at a Wheel of Pain depending on the level of the mechanism and the Thrall itself.

How To Get The Wheel Of Pain In Conan Exiles?
Three types of Wheel of Pain can be unlocked after acquiring their adjacent Knowledge or crafting blueprint. The first type of Wheel of Pain, the Lesser Wheel of Pain, can be crafted once you’ve obtained the Thrall Taker Knowledge, which will give you the blueprints for Fiber Bindings, a Thrall Pot, and most importantly, the Truncheon.
With the blueprint on hand, you’ll need to farm specific materials required to begin building the mechanism, which is as follows:
- Wood x200
- Stone x200
- Iron Bars x50
- Twine x50
After completing the build, you can place the Lesser Wheel of Pain anywhere near your homestead, for which the next step is to find an NPC to convert. If you’re starting in The Exiled Lands, you’ll need to begin with Tier I to III Thralls, for which you can find many spawning in and around The Summoning Place.
Before setting off to find an NPC to convert, you must have crafted the Bindings and Truncheon, as the latter will help knock them out without killing them. Use the Bindings to tie the NPC before bringing them back to your home, and you can place them on the Wheel of Pain to begin converting them.
Don’t forget; you will also need a bonfire or campfire at home to cook Gruel, which can feed your newly converted Thralls on the Wheel of Pain. Depending on the type of Wheel of Pain, this can influence how long it will take to convert the NPC to a specialized Thrall, and once they’ve been converted, you can place them in your Inventory, in the world, or at their dedicated crafting station.
Crafting and using the Wheel of Pain is one of the steps to be completed for the Thrall Taker Journey. Once this Journey has been accomplished, you’ll receive the blueprint to craft the concussive item, the Purple Lotus Orb.
