Can you become a Vampire in Baldur's Gate 3?
You can (sort of) become a Vampire in Baldur's Gate 3. If you progress through Astarion's story enough to get him to defeat Czador and ascend to become a full-blown Vampire Lord, then you can let him bite you. This will turn you into a Vampire Spawn, which is a much less powerful version of a Vampire Lord, but still has the same abilities that Astarion has throughout the main game.
It is worth noting that allowing Astarion to ascend is very much classed as the Bad End to his story, as evidenced by the music that plays when he does it and the way that he acts after he has actually ascended. He's also literally slaughtering thousands of people to do it, so don't do this if you're going for a morally good playthrough.
There's no option to become a full Vampire Lord in the game, sadly, meaning you can't do what a Vampire Lord can do like turning into mist and just entirely drifting through the air, but you can at the very least bring your health back up using the Bite ability.
